Referral Information
Referrals to the Bath Radiology Group can be made by downloading and completing the referral form below.
Referral Form Download
Your GP, Hospital Consultant or Health Practitioner can refer you for an examination by completing the referral form, which can be sent to us by fax or E-mail.
Please note that a request form can only be accepted if the referral has been made by your GP, hospital clinician, or by a health practitioner who is registered to refer patients to us for diagnostic imaging. Your doctor is requested to provide their GMC number to verify this request. Other health practitioners are requested to provide a contact number for verification purposes if required.
